Guinea worm.

Niggers drink the water, infected with the worm's larvae. Then the worms hatch inside the nigger' body and eventually come out through the skin. Some can be as long as a metre :puke.

The bad news is that its not fatal and could possibly be eradicated soon, although some remote nigger areas are difficult to treat.
